Paul Chiang, MD, CMO, Home Centered Care Institute
Paul Chiang, MD is Chief Medical Officer of the Home Centered Care Institute. The Institute’s mission is to expand home-based primary care practices and the workforce nationally through training, research and advocacy. Dr. Chiang is also the Medical Director of Northwestern Medicine’s HomeCare Physicians which has provided over 110,000 house calls to more than 8,000 medically complex patients over the past twenty years. Dr. Chiang has personally made over 32,000 house calls. Dr. Chiang previously served as the Medical Director for Northwestern Medicine’s Home Health and Hospice where he started a home-based palliative medicine program. His work has been featured on ABC News Chicago, WTTW public television Chicago, and in several Chicago newspapers.
Connie Christopher, RN, BSN, Director, Care Transitions, FirstHealth of the Carolinas, Inc.
Connie has been the Director since July 2018. Prior to July 2018, she had held the position of Associate Director since 2007. Ms. Christopher has over 20 years of home care experience. She serves as the overall leader and manager of FirstHealth Care Transitions which includes: two freestanding not-for-profit home health agencies; a complex care management program for patients not eligible for home health but who require ongoing support and management of their chronic disease; and the FirstHealth Center for Telehealth providing remote monitoring services across care settings. She is the co-developer of the FirstHealth clinical pathways of care for heart failure, chronic obstructive pulmonary disease, diabetes, cardiothoracic surgery and hip and knee replacements. Additionally, Ms. Christopher assisted in the development and implementation of a comprehensive competency program for all staff that includes initial and annual competencies and targeted milestones.
AlexSandra Davis, RN, BSN, MPA, Director, Home Health Services and Director, Community Care Network Wooster Community Hospital
AlexSandra is a registered nurse with 46 years of experience. She holds a BSN, and MPA in healthcare from Cleveland State University college of business and a certificate in practice management. She has had many roles in healthcare from being a cardiovascular nurse to program development. Her latest accomplishment was the implementation of the Community Care Network, a collaborative program with The College of Wooster. This program trained students to become health coaches. In addition to being the director of the Community Care Network, she is also the director of Home Health Services at Wooster Community Hospital in Wooster Ohio.
Lisa Remington, President, Remington Health Strategy Group, Publisher, The Remington Report
As President of the Remington Health Strategy Group, and Publisher of The Remington Report magazine since 1990, Lisa has worked with over 6,000 home health, hospice, palliative care, private pay and community-based organizations in both a consultancy role and educator. Her expertise drives organizational “value” planning to increase revenues, identify new and existing growth opportunities, and leverage strategic partnerships across the continuum. Nationwide leaders and their teams attend Remington Think Tank summits, executive leadership programs, and peer-networking groups to improve performance, hardwire their organization’s future, and lead with vision. Lisa monitors the complex key trends and forces of change to develop a correct strategic approach to de-risk decision-making and create sustainable futures.
Karen Thomas, President, Cardeum
Karen is President of Cardeum, a Health Care Company specializing in innovative solutions and technology. Previously, Karen served as President of Advanced TeleHealth Solutions, a telemonitoring company, and Oxford HealthCare, a home care agency. In 2004, Karen developed and secured funding for the first Medicaid covered telemonitoring program within the Disease Management Department for the State of Missouri. Seeing the need that various industries had for cost-saving solutions that could provide improved health outcomes for patients, she began Advanced TeleHealth Solutions in 2011. She is a speaker for Missouri State University’s Health Care Administration Masters program regarding “The Capabilities of Home Health and Its Future,” a contributing author to Home Telehealth: Connecting Care Within the Community, published by Royal Society of Medicine Press Ltd.
