Accurate information supports improved outcomes. Because many patients receive care for multiple conditions and see different providers, it can be challenging to coordinate all the details of a patient’s health history.

Keeping track of medical records including various diagnoses, treatments, allergies, medications, doses, and more shouldn’t rely on a patient’s memory or require carrying copies of medical records from one provider to another. And in an emergency situation, access to accurate information—especially if a patient is unable to provide it—can be crucial in making care decisions.

Without real-time medication capabilities what we see, especially in a home health type of environment is that the home health provider goes out and meets with the patient during intake and simply has to collect all this information manually from the patient.

This includes talking to the patients, a lot of times going through their prescriptions, talking to family members, or reaching out to providers. And as we know, in many cases, this information is incomplete. If a patient decides they want to discontinue a medication, because they didn’t like a side effect, they stopped due to cost.

We’re not going to collect all that information we need in order to effectively treat this patient. And as we know, when they have an acute incident, they go to the hospital. Oftentimes they’re going to come out discharged with a different medication list or medications than they have in their home.

The additional larger problems that result, of course, are errors of admission, duplicate medications, drug interactions, all of which, of course, can contribute to that larger adverse health event episode.

From a referral source, they have just a very thin sliver of care history on that patient. They may drive out to that patient’s house, discover a shoe box full of medications, and they don’t know what that patient’s supposed to be on.

They don’t know what the rest of their problem was because they weren’t properly informed. So what does that clinician do? They spend the next couple of hours on the phone, talking with a referral source and talking with other specialists trying to reconcile that medication list, trying to complete the picture of what’s going on with that patient. And in the meantime in home care, they are not spending that time with the patient. They’re spending it on the phone. And then also in terms of hospice based care, that’s the toughest day in the life to the patient and their family members who are present.

We really want to leverage the right set of tools so the clinician interacts with the patient in the way that a natural caregiver relationship is supposed to work.
