In home-based care, the skills gap is widening, while patient care needs increase and become more complex. Multidisciplinary teams can work collaboratively to increase productivity and improve patient outcomes to achieve a patient-aligned care team. To sustain care quality and patient outcomes, providers should explore team-based staff models that co-invest in workers’ capabilities and support clinicians working at the top of their licenses.

High-functioning clinical teams are essential for the delivery of high-value healthcare and have been associated with decreased workloads, increased efficiency, improved quality of care, improved patient outcomes, and decreased clinician burnout/turnover.

In this webinar, we explore:

  • Best practices to improve clinical productivity in team-based care.
  • RN and LPN team staffing strategies.
  • Therapy and rehab staffing models.
  • How to develop roles for professional ancillary disciplines within the home-based team model.
  • Vertical and horizontal career development to improve staff engagement and promote retention.
