2025: Medical Conditions that are Significantly Reducing Hospital Readmission Rates

Hospitals across the United States have seen a significant decline in readmission rates for the third consecutive year, marking an important achievement in healthcare delivery and patient management. For hospitals with the lowest number of dual-eligible patients, the average penalty is 0.32%. During fiscal year 2024, those two groups were assessed penalties of 0.29% and 0.34%, respectively.
