In the midst of extreme healthcare reform, we know that redefining clinical continuum relationships must occur. As outlined in a recent Remington October 14, 2014 executive briefing webinar, key strategies to accomplish clinical continuum includes reducing the overall cost of care, reducing referral and partnership costs by aligning quality and clinical measures, and developing new post-acute tools for redefining care continuum partnerships.
